RESUMO

La histerectomía es uno de los principales procedimientos quirúrgicos en ginecología. Se calcula que en EE.UU. se realizan unas 500,000 anualmente y que una de cada nueve mujeres a lo largo de su vida se realizará dicha intervención. Los distintos abordajes (abdominal, vaginal, laparoscópico y robótico) presentan diferencias respecto al tiempo quirúrgico, complicaciones, dolor postoperatorio, estancia hospitalaria y gasto sanitario. Factores como el tamaño uterino, la accesibilidad vaginal, la historia de cirugías pélvicas previas, la existencia de patología extrauterina o la experiencia del cirujano influyen a la hora de decidir la vía de abordaje. Revisar la evidencia disponible respecto a la vía de elección de la histerectomía por patología benigna y cómo decidir el abordaje más adecuado para cada paciente. Búsqueda bibliográfica de literatura en las bases de datos PubMed, Medline, Embase, BioMed Central y SciELO. La vía vaginal es el abordaje de elección para realizar una histerectomía dada su menor invasividad, menor tiempo de recuperación y menor tasa de complicaciones. La generalización del uso de algoritmos para la elección de la vía de abordaje incrementaría la tasa de acceso vaginal y asociaría con ello una disminución del gasto sanitario.


Hysterectomy is one of the most common surgical procedures in gynecology. It is calculated that over 500,000 hysterectomies are performed in the USA per year and that 1 out of 9 women will undergo this surgery in their lifetime. Diverse surgical approaches are possible (abdominal, vaginal, laparoscopic and robotic) with differences in operative time, complication rates, postoperative pain, hospital stay, and health care cost. Factors such as uterus size, vaginal access, prior pelvic surgery, extrauterine pathology and surgeon experience may influence the route selection. To collect the current evidence regarding the preferred route in hysterectomy for benign pathology and how to select the adequate approach for every patient. Bibliographic literature search through the PubMed, Medline, Embase, BioMed Central and SciELO databases. Vaginal hysterectomy is the preferred approach in benign indications. It is less invasive, and it shows shorter operative time, shorter recovery time and less complications. The use of decision algorithms to select the hysterectomy route may increase the vaginal approach and decrease health care costs.

AIM: Abdominal cavity access accounts for 50% of complications during laparoscopic surgery. Different safety maneuvers have been used to try to diminish these. Our study aims to establish the usefulness of Palmer's test in the correct positioning of the Veress needle and the reduction of complications during laparoscopic access maneuvers, when used in addition to the determination of intraabdominal pressure. METHODS: Prospective observational analytic multi-centered cohort study with 370 patients undergoing gynecologic laparoscopy between July 2014 and November 2019, comparing the additional use of Palmer's test in 185 patients (Palmer-Test-Yes, PTY), with intraabdominal pressure determination alone in 185 patients (Palmer-Test-No, PTN). RESULTS: Intergroup homogeneity was described for the basic characteristics of both population samples, except for mean age and percentage of previous laparotomy. A total of 19 complications were recorded, 10 in PTY and 9 in PTN, with no significant differences (P = 0.814). No differences were found in the analysis of these complications, except for the rate of conversion to laparotomy, which occurred four times in the PTY group and none in PTN (P = 0.044). Furthermore, no differences were found once fixed for the history of previous laparotomy (P = 514.), nor for the percentage of successful access after the first attempt between both groups (P = 0.753). CONCLUSION: Palmer's test, when used in addition to intraabdominal pressure determination, has not shown to be effective in preventing failed access to abdominal cavity or reducing complications associated with access maneuvers with the Veress needle. Hence, its systematic use is not justified, since it could generate a sense of false security.

Resumen ANTECEDENTES: Los leiomiomas son tumores benignos que se originan en las fibras musculares lisas. La mayor parte de los leiomiomas del aparato genital femenino se localizan en el útero. La localización extrauterina es excepcional, la incidencia del leiomioma vulvar es del 0.07%. Estos tumores del músculo liso suelen ser indoloros, de crecimiento lento y pueden aparecer en mujeres de cualquier edad. OBJETIVO: Reportar un caso de leiomioma vulvar que, por su baja incidencia y similitud clínica con otras lesiones, como el quiste de la glándula de Bartolino, pueden conducir a un diagnóstico erróneo inicial. Además, efectuar una revisión bibliográfica que aporte conocimiento a su diagnóstico y tratamiento. CASO CLÍNICO: Paciente de 52 años, con una masa vulvar de años de evolución y un diagnóstico equívoco inicial de quiste de la glándula de Bartolino. La paciente fue intervenida mediante escisión quirúrgica y estudio histopatológico posterior. El diagnóstico definitivo fue de leiomioma vulvar con diferenciación simplástica. CONCLUSIONES: En contraposición con sus homólogos uterinos, la localización vaginal del leiomioma es excepcional. Si bien hasta ahora no ha podido demostrarse la efectividad de la biopsia preoperatoria se aconseja su toma porque podría tratarse de una masa de características malignas, lo que modificaría el tratamiento y su urgencia. La actitud quirúrgica es, quizá, la más acertada porque permite, además de un diagnóstico anatomopatológico definitivo, un tratamiento curativo. Se propone la escisión completa, mediante una incisión elipsoidal en la piel y asegurar la extirpación completa de la pseudocápsula.


Abstract BACKGROUND: Leiomyomas are benign tumors that originate in smooth muscle fibers. Most leiomyomas of the female genital tract are located in the uterus. Extrauterine localization is exceptional; the incidence of vulvar leiomyoma is 0.07%. These smooth muscle tumors are usually painless, slow growing and can occur in women of any age. OBJECTIVE: To report a case of vulvar leiomyoma that because of its low incidence and clinical similarity to other lesions, such as Bartholin's gland cyst, may lead to an initial misdiagnosis. In addition, a review of the literature should be carried out in order to contribute to its diagnosis and treatment. CLINICAL CASE: A 52-year-old patient with a vulvar mass of years of evolution and an initial misdiagnosis of Bartholin's gland cyst. The patient was operated by surgical excision and subsequent histopathological study. The definitive diagnosis was vulvar leiomyoma with symplastic differentiation. CONCLUSIONS: In contrast to its uterine counterparts, the vaginal location of leiomyoma is exceptional. Although the effectiveness of a preoperative biopsy has not been demonstrated so far, it is advisable to take a biopsy because it could be a mass with malignant characteristics, which would change the treatment and its urgency. The surgical approach is perhaps the most appropriate because it allows, in addition to a definitive anatomopathological diagnosis, a curative treatment. Complete excision is proposed, by means of an ellipsoidal incision in the skin and ensuring complete excision of the pseudocapsule.
